Story

Perspective: A patient wanting to send information to her provider
 
  Context: The patient has made the decision to disclose her PHI to the provider. The provider has previously assured the identity of the patient and has assured herself that the address is owned by the patient.
 
  Story: At some prior time, the provider "whitelists" the patient's address. After that, the patient composes a message to the provider, and attaches health information (e.g., seizure diaries, glucose logs, pictures of a wound, etc.), and sends the message to the provider. The message is received by the provider's EHR and displayed in an inbox queue for the patient.
